I'm hoping for some guidance. We live in Washington state and have struggled to find good help for my wife.
My wife was born with Hirschsprung's disease, but wasn't really cared for as a kid and dealt with the side affects all the way through college.  She received a J-Pouch as a youth adult and that seemed to be a game changer for her.  She has dealt with bowel obstructions on and off, but generally was able to manage everything. 
Recently she has been diagnosed with gastroparesis and at the same time, inflammation in her small bowel and ulcers and fissures with the J-Pouch.  All of these things together are causing extreme pain.  She has now been on 6 weeks of antibiotics and 5 weeks of steroids, but has seen no improvement.
We are looking for someone or some group that may be able to help her.

Wow, sorry you guys are having to deal with this, and feel like there are no useful solutions. Hopefully, someone with some more specific experience will come along. It seems that your wife’s colonic disease also affects the upper GI system. I presume you have sought out experts in motility disorders in Washington. I have heard of people traveling to the Mayo Clinic or Cleveland Clinic for comprehensive work ups in these complicated cases, but there is not always lasting results.

One question I have is regarding her fissures. Are they anal fissures, or in the bowel walls? Anal fissures can be excruciatingly painful, and usually topical treatments are best, such as topical lidocaine and/or nitroglycerine. If she has anal fissures and can get those properly treated, it might make all the rest tolerable.
