Just wondering options. I have jpouch. They found 2 very narrow strictures in terminal iliem and ulcers in terminal ileum. They dialated both to a 16 but said chronic severe inflammation in most recent biopsies. I've changed my diet so noticed less watery stools, less urgency...feel better overall but not perfect and biopsies still not good. My gi doctor at Cleveland clinic (who is pretty new), said that she would recommend entyvio to control inflammation. I initially thought no drugs but I'm concerned about these strictures causing a blockage. Just wondering if people here pro meds to control inflammation or has anyone had luck controlling re occurring stricture growth....and would suggest just to monitor and keep meds out of it?

I didn't have a stricture but chronic inflammation up into the terminal ileum for many years. Chronic antibiotics cut it down a lot from 2020 on. I also tried several biologicals before, but none of those helped me.
Until I changed to Skyrizi last December, that has a really good effect for me. But everyone is different there, so you will have to try. Anyway I would recommend getting inflammation under control.
You really ought to manage that inflammation, if you can. Diet alone probably won’t be adequate.
I have been dealing with UC since 1972. The concept of “no meds” has been pretty much of a fantasy for me. Maybe some folks can control some symptoms with diet and lifestyle, but my experience is that does not work in the long run. I started out with pancolitis when I was 15 years old and eventually had nearly 2 decades of remission with only an occasional flare and intermittent medication treatment. Then it all blew up and I wound up with a j-pouch. I had another run of some med free years before I was saddled with enteropathic arthritis, then chronic pouchitis. Now I am on Imuran, Inflectra, and sulfasalazine to control my IBD.
The moral of the story is that IBD is random, episodic and not predictable. Personally, I am grateful to be living in a time of so many options!

i have had a pouch in 2008 with alot of structural issues and then a Remzi redo in 2015. I deal with minor leakage from time to time since my redo which Remzi stated redo with mucousectomy is a 50% chance of having some leakage.
but for the past several years i have had 2 small ulcers in mid pouch that just havent cleared with antibiotics but not really causing any issues. I am still with cleveland clinic and my GI has not talked about biologics at all. I dont want to go on them unless necessary. I would try maybe Budesonide before trying biologics but i dont have any stricture issues so you are a little different.
