Hi all!

I guess I'm wondering if it's possible to be nursing a partial blockage, without the pain or nausea you'd ordinarily expect, for four, maybe five at this point days. I've had my j-pouch for a year and four months due to FAP and, until last Friday, I had to use the restroom every hour and a half, was working on getting that to two hours or more and adding Metamucil in with some success. That day, or maybe the day before (in the interest of full disclosure), I smoked some pretty low potency marijuana after two meals, and abandoned it Friday after I suddenly had no movements from, like, 10 AM to 5 or 6 PM.

At that point I called the hospital I get GI care at to ask them what to do. The doctor on call said he'd never heard of constipation with a j-pouch but also did not think, without pain or nausea, that what I had was a blockage. He recommended I use MiraLax, which I did, (he also suggested I continue using Metamucil, which struck me as counterproductive?) and skipped dinner. That seemed to do the trick over the course of the night so, on Saturday, I didn't continue taking it. My bowel seemed to be producing stool infrequently and in small amounts but I felt at that time that continuing to use MiraLax wasn't a great idea in terms of getting back to normal. I went to bed without dinner again, sort of wired, and woke up on Sunday morning to take some MiraLax again. It didn't really seem to do anything so I muddled through the day and made it to the end, without dinner, with an additional dose of MiraLax after an additional call to my hospital led to the on-call doctor telling me I could take it two to three times a day.

That once again did seem to clean me out at some point early in the morning so on Monday I once again didn't take any MiraLax and muddled through the day. Now I find myself once again in the middle of the day, with a bowel pattern that does not match my normal at all, wondering what exactly is wrong with me. I've had a blockage before when I had my ostomy so I'm familiar with the pain I apparently ought to be in by now and the nausea if I were blocked but it doesn't sound like people with j-pouches get constipated either. I'm about to take MiraLax again because I can't think of what else to do. It's starting to feel, however, like I'm in some kind of tailspin I can't stop that just doesn't make any sense to me. I'm certain skipping dinner isn't doing me any favors, reasonably sure that continuing to take MiraLax is going to make it hard to determine when I'm back to normal at all and it doesn't seem to be fixing the issue, and I really just can't tell which thing I'm doing wrong (if anything) has caused or is causing this problem or if I'm aggressively trying to over-manage something.

I apologize for the long, rambling post; I guess I was just hoping I could get some clarity on what it is I should do or what, from other pouchers' experience, this would even be. I doubt it will be an emergency if I can just clear myself with MiraLax again but I'm not getting to any kind of fix and it's very frustrating when I was already afraid that I'm moving too late on trying to thicken my stool and get somewhere comfortable with how often I have to go. I appreciate any insight because, over a year into this, I don't feel like I have much insight into life with a j-pouch myself.

A little mirilax goes a long way.

And it takes 8-10 hours to work.

I don't have a pouch. But I know the feeling of a block. If I'm certain I take a small amount of mirilax. But it takes a long time to work.

The last time I used it four hours after taking mirilax I had a full block and it took four plus hours for the mirilax to work. And thank goodness it did.

Your problem reads like something else. I may be wrong. Usually am. Lol.

Have you tried a heating pad? As high as you can stand it without burning yourself.  That has worked for me.

For what it's worth, that's what I think too; I logically really would be in pain by now if I were blocked, I think, and I remember the feeling better than I'd like. I guess I'm just confused about what caused my bowel habits to shift the way they have and concerned that I can't seem to course correct. It's possible that the way I reacted and have kept treating it is confusing things. I'll definitely try a heating pad out! I haven't yet but I'm willing to try anything out that works. Thank you for the suggestion!

If you’re having no pain then a blockage really is unlikely. The words “constipation” (and “diarrhea”) are used here to mean so many completely different things that we’re better off without them, I think. If your only symptom is reduced frequency then perhaps you’re just fine? Stay hydrated, of course. Metamucil/psyllium tends to get the stool to the right consistency for a J-pouch.

Hi, @itaintraito - 18 years ago I had a colon resection and  jpouch surgery to remove most of my large intestine due to polyposis (not FAP, just 40+ polyps discovered at my first colonoscopy, go figure).  After 16 years of managing diarrhea, for these past 3 months I have been managing constipation. It's quite the unexpected change. I drink Miralax daily, use an enema as needed, sometimes take senna or dulcolax, and probably most importantly, have tripled my water and liquid intake. 

I have bunch of diagnostic work scheduled for the next 2 months to try to figure out the "why" of this persistent constipation. Having had a half dozen hospitalizations for bowel obstructions, I appreciate your concern about blockage, but the waves of extreme pain accompanying an SBO are unmistakable so it doesn't sound like you are dealing with that. Good luck, do let us know how you are doing if you have time to post.

Hi @SeattleJane. I'm sorry to hear you're in what sounds like a boat similar to mine. It hasn't been fun for me so I can only imagine what three months of it has been like for you. I'm doing about the same as last time I posted but have stopped skipping dinner; I can't imagine that was doing me any favors. I also still couldn't really help but take MiraLax out of what I guess must be an abundance of caution.

Tripling water intake sounds like a reasonable idea. I've started to think water intake is probably one of the issues I'm having. I'm not certain I'm drinking enough (I'd say I land somewhere between 64 to 80 or so fluid ounces a day though that hasn't seemed to be a problem before other than what has always felt like mild difficulty staying hydrated) and it just seems to go right through me. It seems as if I end up having to urinate every half hour to an hour. I've added in electrolyte powder mixed into a glass or Gatorlyte once or twice a day to try retaining it better. Dunno if that's the MiraLax in my system doing its job or not or if I'm simply not getting enough electrolytes.

I guess, at this point, what I'd say is that I'm not worried, cautiously optimistic that eating regularly and getting hydration right should clear this up as best as I can understand it, but a little frustrated and antsy. Thank you for asking after me and for the reassurance; I feel somewhat out of my depth right now and do worry a little over whether or not I've treated my j-pouch with the appropriate care or habits or if I've done something wrong that I can't course correct for. But that's been sort of typical for me since I had my pouch completed.
