Hi, I had got operated ( 3 stage surgery) in 2017, I am in no medication post surgery . Have had occasional blockage and pouchitis issues , but manageable . Of late I am always facing a burning sensation while passing stool and the no of times of urge to pass has increased significantly. If anybody has experienced this , please could you share how you have got over of it . Any medications taken. My doctor is not of much help till now for this problem. Eager to get solutions as it is pretty painful and draining

Instead of trying creams in the same family, I would try Calmospetine. I tried other creams in the same family too but Calmospetine is the only thing that truly works for me. I would give Calmospetine a try.

Hemu, I have been on antibiotics continuously for 16 years. Most folks with pouchitis can clear up the occasional case with a simple 14-day course of Cipro or Flagyl. Some of us, though, have what’s called “antibiotic-dependent pouchitis.” In those cases pouchitis symptoms invariably return shortly after stopping antibiotics. These days some of us still stay on antibiotics, and some switch to a biologic medication (there are now numerous choices). If my antibiotics fail I will likely make that switch.

Since your frequency seems to be increased it’s possible that your pain is simply caused by that. In that case I’d guess pouchitis to be a bit more likely than cuffitis.

If the pain happens mostly while passing stool and afterwards, then you may have to consider a fissure at the anus also (often a little inside so you can't see it and it might be mixed up with inflammation of the cuff). If some particle gets stuck in a fissure during a BM, then there is a burning sensation for quite some time, which is very uncomfortable especially at night.

If it feels more like a painful pressure from inside which makes you go to the toilet more often, then a cuffitis (+ pouchitis?) is more likely.

This is basically identical to what I go through. For the last couple years the issues I have been dealing with have not been fully diagnosed by my Dr. I will start having more frequent BM's, urgency, little amount of stool each bathroom trip, pain in the inside of anus, this pain always seems worse at night. Basically what I think is happening is I have some form of chronic pouchitis that leads to inflamed anus and pouch, i will have terrible pain and sometimes i will develop a fissure. I have been taking Cipro routinely, which helps a lot, I am also prescribed Nifedipine for the fissures or if im having pain after a BM. This combo seems to be working pretty good. After being on Cipro for at least 6 months, im thinking about maybe changing to probiotics or flagyl and see how i do with those. Im praying it can be managed w probiotics but im certainly not banking on it. Flagyl has less side effects, mainly that it doesnt cause potential tendon tears. Im looking to start BJJ and the last thing i need is a torn tendon, I need to get healthier. If worse comes to worst and probiotics and Flagyl dont work for me I will have to just bite the bullet and stick w Cipro. The Dr. has always talked about calmoseptine like its a cure all but thats for external stuff..The combo of antibiotic w nifedipine has worked pretty good. I also have a bidet and pat myself down w a towel instead of toilet paper (when i can).

my doc was of the opinion it will surely help for outside skin portion, since in my case the burning sensation is just inside layer of the anus he suspects occurence of fissure and has advised me to use a cream Anobilis to be applied inside

if this does not work he will do a sigmoid scopy

he has also given some medications to control the urge to pass

i see immdiate result , no of times controlled, cream applied a couple of times has shown reducing trend in burning sensation

Keeping fingers crossed that it would work

Once I stabilise i will pay a visit to my surgeon to take a complete look

For fissures nifedipine has worked wonders. Basically when you get a fissure in that area all of the tissue swells and becomes inflamed. Because it's so inflamed it can't heal. Basically the Nifedipine allows the tissue to relax so it can begin healing. For the first year I had this horrible pain and the dr sort of kept blowing me off until I said we need to fix this bc something isn't right and my quality of life is suffering. Once I was given nifedipine it was instant relief and took about a week for it to heal. Now I use it on a regular basis when I feel a fissure might be coming on.

I looked that up as well! And it looks great!!

Mine has lidocaine, nifedipine and hydrocortisone. I can barely use it due to extreme screaming pain every time I apply it. No one else is mentioning the intense pain of med? Maybe my % of lidocaine is too high. 5%. Have used for 5 days and minimal improvement in that area. Actually seems to heal without output  then a BM sets it back.Have had somewhat  reduced frequency, likely due to taking Cipro. Arghhh!

That sounds terrible!! HAve you tried Calmoseptine???

Seeing Colorectal surgeon Wednesday where I live and driving to  CC Thursday for consult there. Maybe (hoping) to get suggestions on how to move forward!

Yeah some people cannot take that menthol, its rough for some people unfortunately. You probably have very sensitive skin which can be a good thing too.

I seem to have a similar issue, pain in the anus region, small amounts of stool per trip, feeling pressure inside the anus (not inside pouch I believe). Even a smal bubble of air inside me forces me to visit the loo. At night it is worse than during they day. I was prescribed Budesonid (3mg, 3 times a day) and Mesalzine sups (2 times a day). After having taken that medicine for one week now, I start feeling better.

I get that sometimes.  I think it must be a combination of things, but mostly I wonder if internal hemorrhoids are causing this problem?  I do not have a rectal cuff, or I would suspect cuffitis.

I followed advice of I think Ken who compounded Coloplast Bravo paste with  with Cerave healing ointment 1 part paste to 4 parts cerave and it’s almost as good as ilex paste. Wow! Finally I’m healed. I also use script from Dr with Nystatin/Cholestyramine/ white petroleum that can be put inside anus. Remarkable! After months of pain I’m healed though my acidic Nike output keeps coming back. At least I can protect it with these products. Such relief!  Thank you Ken for recommending this combo! I am grateful beyond words!

Where exactly do you apply the compound?  Is it these two things? They have variations so I just want to make sure I get the ones that work:

Is nifedipine a pill or a salve?

Those are the 2 products. No sting- thankfully! Had initially used Convatec Stomadhesive but it stung. Initially did 60/40% but I reduced amount of paste. Mixed well. Good luck! I push a dab up into Amy’s and also put plop right at anus. It doesn’t harden as much as Ilex did but best of anything I’ve tried. I do have rectal cuff but  had tear and ulcerations inside and around opening. They have healed!

Thank you!  I may try that out sometime! I usually use Calmoseptine like Lauren does, but I have wondered if there is anything better because the Calmoseptine comes off when you poop and wipe. When you said "up into Amy's" I imagine you were using speech to text and you actually said "up into the anus". Is that correct? So you put it a little ways in there. Good to know that isn't irritating.

I'm so glad you have healed. Yay! And thanks for the tip! I get a sore or itchy butthole from so many different things.  Yeast...butt burn...hemorrhoids!  It's hard to tell which thing is causing the irritation, and therefore, which remedy to use. I wish I had a test that would tell me.
