Hey everyone, I’m experiencing something new and hoping to hear from anyone who has had a similar experience. I’ve had my pouch for a little over 5 years now and most of my years have been great other than the occasional speed bump. I’ve had pouchitis a few times and am currently on day 4 of Cipro (500mg twice a day) for some mild pouchitis. I was excited (it’s weird what we get excited about these days huh?) leaving my Dr’s office the other day after he did a quick examination and told me this is looking like a very mild case of pouchitis and two weeks of Cipro should do the trick, as it usually does. Although my pain and energy have improved a bit, I’m worried because I am still having symptoms of pouchitis after 4 days on Cipro.  In the past, I’ve improved much quicker and know that Cipro is suppose to work very quickly for most.  My symptoms right now include 8-10 very loose bowel movements a day, still some cramping with lots of gurgling, mild urgency, nighttime incontinence/spotting, and an overall feeling of the inflammation still being present. Has anyone ever had Cipro kick in later than usual? Should I give my doctor a call to add in another antibiotic?

Im of course programmed to worry at this point, so I’m hoping the Cipro starts working soon, especially now that I’ve added in a daily turmeric supplement as well as VSL #3 probiotics in between doses. VSL has always worked well with my system, but I’ve slacked pretty hard on taking it regularly over the past few months which may be why I’m having issues right now.

I would give it another couple days, and then switch to another antibiotic. You can build up a resistance to any antibiotic you have taken for a while, which is why it's a good idea to rotate. 4 days should be more than enough time to notice abatement of symptoms, but if it does not improve I would either switch to or add 1000 mg flagyl. For me cipro worked best in tandem with flagyl. Augmentin is another good one. Good luck.

Cipro alone worked well for me for about 6-7 years, in my case taken daily for chronic pouchitis. When it lost effectiveness I got good results by adding Flagyl (Flagyl alone had never worked for me). That combination has been effective for about 8 years now.

It’s reasonable to give the Cipro through the weekend, but I suspect you’re going to need to add something.
