I’ve had my j-pouch for 19 years now and I’ve had no problems at all. After taking Augmentin for a sinus/ear infection, I am now bound up - this is a first for me. I’m still passing gas occasionally but experiencing anal spasms that make it feel like I need to go…but then nothing (or very tiny bits) come out. I didn’t eat for the last 24 hours, and about 8 hours ago I chugged a grape juice with miralax. I was able to pass a little bit of stool overnight last night but I can tell I’m not emptied out. I’m trying not to push but occasionally a very strong spasm happens and it feels like a bowling ball covered in spikes is trying to exit my bum 😩

On to the questions… when do I need to look for medical help? How long will the miralax take to work? Any other advice?

This is a bit like a blockage but at the end of the pipe. I having trouble thinking of a reasonable model of how Augmentin might lead to anal spasms. Perhaps your stool got too hard and you strained excessively to empty? Did you produce any bricks?

You might want to try a warm tap water enema, to help break things up and get some more information (e.g is there an anal fissure? that can cause spasm). Keep drinking plenty of clear fluids, and maybe back off food until things are flowing again. MiraLax will eventually work in all likelihood.

If you can’t stay hydrated you’ll need an IV. If you develop severe pain or fever you should get to a hospital.
