My pain is currently off the chart. I am screaming in pain and I am pretty tough. I have chronic pouchitis constantly on either Flagyl or Augmenting for two years. For six months I have had every night incontinence 2 to 3 times and daytime urgency and sometimes incontinence. I am having up to 20 BM/day.

The pain today is severe lower left abdomen cramping and pain and tenesmus is constant. The BMs as they are about a few inches I guess up the anus hits an exquisitely Pai full spot. I can barely breathe through it. Of course, the wiping so many times a day leaves the anus so inflamed. I do not have a bidet nor will a portable one fit in our tiny bathroom. I have a spray bottle, but the water kills more than the paper right now. Please help.

Thanks for your reply. I had scope of my pouch yesterday. I am swollen and inflamed throughout and am being sent to GI for biologic discussion or have a permanent ileostomy which I am okay with. I worked for a rheumatologist for years with many patients with autoimmune diseases being on a variety of biologics.  I saw too often the down side of severe side effects and am hesitant. I also know many with a Jpouch who did not respond well to biologics with regards improvement of symptoms. My incontinence is getting the better of me mentally and my social life and sports suffer greatly. I have 3 BMs every single overnight; and I mean full BMs, not a little leakage. Anyways, just tired.

I hope doc did not miss a leakage  I have no fever, just very crampy. For now, I will just continue with Flagyl as it is all I've got. I will go to emergency if this G's escalate.

About 3 years now and I frankly can’t tell that I’m taking it as far as side effects.  So, none.  It’s controlling chronic pouchitis.  I did Humira for several years prior, which was easier as it’s an at home simple injection, but it stopped working after many years.  Entyvio is infusion, so it’s a little more of a hassle.  Although I just saw that there’s a company now that will come to your home and do infusions right there.
