I have always used A & D Ointment (can be found in the baby isle). Be sure to clean yourself well after each BM. Then apply the ointment. I use unscented personal wipes or sensitive baby wipes. Also, as time goes by you shouldn’t be going as many times a day. That will help too.

20 times a day sounds bad.after my takedown I needed to use a travel bidet then toilet paper and calmoseptine and a hair dryer after every bm for the first couple  of months but now 18 months later I just go normally with toilet paper and sudocreme after showering once a day and change my underwear twice a day to stop jock itch.i usually only get butt burn at Christmas from too much chocolate and eating,drinking.

things I wish I did earlier from my takedown. Bidet for toilet, travel bidet baby wipes etc. To help with butt burn get meta mucil wafers. It will help bulk your stools and help control the burn. I personally stay away from any cremes or ointments after I developed a branching fistula a year after takedown.

Try and find a way to manage with fiber and Imodium and stay clean.

I feel your pain. I'm 7yrs post colectomy and still have diarrhea 10 or more times a day.   butt burn so bad and constant that I am considering getting colostomy. I use Ilex paste all day. Always have some on anal area. Only product that works. Acidic fecal material just immediately breaks down other products allowing contact with skin. This product is not perfect but the best I have found. Some foods add to problem but I really can't tell much difference with diet change. Sorry I dont have better news. I am looking for help too. Dont feel bad---this is not a matter of cleanliness. Diarrhea is uncontrollable and the burn happens immediately. It is not like we walk around with messy bottom all day!  Also a bidet helps. Inexpensive ones work fine.  Good luck!

Have you tried different fiber supplements? I do not get a bulky stool without fiber, some days I use no Imodium and just meta mucil wafers. With just Imodium my stool is watery. Also in my opinion how clean your skin has a lot to do with it. For you not to feel the “instant burn” you have to stay clean and get your skin to heal. Only time I experience bad butt burn is from lack of fiber, spicy foods but it’s tolerable even with spicy foods as long as my skin is well at the time.

Don’t know never had Pouchitis . I’ve had urgency to go since day one . I can hold it tell I get to a restroom , and  most of the time it’s the size of a dime or smaller  I can set  there for 15 minutes nothing more comes out but as soon as I get up and walk five feet or so that urge comes back ? Other than that I feel great

@RodD we’re on the wrong thread, but: when was your last pouchoscopy? DO you have a GI who has an opinion about what’s going on? Can you tell for sure whether you have 1) incomplete emptying (plenty in the pouch that just won’t come out), or 2) an urge to empty a pouch that’s pretty much empty already? These can sometimes be hard to tell apart. Both of them lead to unsatisfying visits to the toilet, but the causes and treatments are very different.

My last pouchoscopy was a week before reversal . I haven’t seen my gi yet my first appointment is on the 9th . I’ve been trying to tuff it out but it’s getting worse not better. All the food I thought I could handle and did handle pretty well , now give me the burns . It’s seem like the only good that came out of this is that the cancer and ulcerative colitis is gone , i bm as much or more than when I had the Disease except now I can and major but burn to it😡

Hopefully your GI will have some sense of what’s going on. Pushing on an empty pouch will just lead to problems. At 5 months in plenty of J-pouchers are still adjusting, What things are you doing besides Imodium? Have you tried psyllium fiber (Metamucil)? I used that from the very beginning, and it helps me. Have you tried gently delaying bathroom trips to gradually “stretch the pouch?” What ointment are you using? Even plain zinc oxide applied after every poop will begin to allow external healing, but some folks prefer Calmoseptine or Ilex. If you’re having spasming there are medications that can help, but I’d be careful about driving after taking those. Good luck!

RodD, welcome to the “club”! Butt burn is normal until you find what works for you. This site has been very helpful, use the search tool.
I’ve had my Jpouch 37 years. Recently I learned that Preparation H w Lidocaine mix w reg Preparation works great for the burn. I purchased a bidet a few years ago. It helps but if you don’t have one just use a squirt bottle to rinse afterwards. I use white washcloths to dry. Taking Epsom salts baths, there are scented ones,  really helps the most! Have a good book to help ease your mind and soak for at least 20 minutes. Finding that eating a low residue diet, very little fiber, helps. Oatmeal is ok sometimes. For a blockage, eating potato soup help & walking & squatting helps, but don’t hesitate to go to ER. They saw me many times the first few years but it was from eating the wrong food.
