Could someone please explain the difference in symptoms between cuffitis and pouchitis?

I’ve had a J-Pouch for 20 years and I have been very blessed with no significant issues until a month ago. Now when I eat, I almost immediately get a sharp pain in my rectum and strong urge for BM.

It could be a total coincidence but this started after I went for several hours “holding it” to the point of it being painful because I had no access to a restroom. I am trying to get an appt. with a gastroenterologist but just wondering if I might have cuffitis.

Anyone have a great doc in Alabama with experience dealing with patients post Jpouch?

Pouchitis, cuffitis, SIBO.

All three share similar symptoms.

I have had cuffitis and SIBO, but not pouchitis. Symptoms I have had include frequency, spasms, blood, and a constant sense of need to go (tenesmus).

I am sure the pouchitits sufferers will respond, but a pouchoscopy with biopsies will reveal the real issues and provide a sound diagnosis for treatment.

pouchitus and SIBO are treated with antibiotics and cuffititis is usually treated with mesalamine either oral or suppositories.

All three are well documented on this site.
