If the colon is removed for the j pouch surgery, what good are biologics?  I’m in the midst of deciding whether to have my pouch removed or not due to constant pouchitis/cuffitis.  My gi has recommended biologics from time to time but I have declined. I tried entyvio, humira, and Remicade before j pouch surgery.  I’m just trying to get as much information as I can before making another surgical decision.  I just want to feel healthy again.

What good? For me, Remicade put my chronic pouchitis in a remission that has lasted for years, since 2016. Prior to that I was on various other biologics for my enteropathic arthritis that developed about a decade after my colectomy and j-pouch procedure. They just did not prevent pouchitis in the long run. While colectomy does dispense with your colitis, you still have the underlying disease in your genetics.

Colectomy saved my life, but did not cure my autoimmune disease. Ileostomy can’t do that either. It can cure you of a failed pouch though. But what if you actually have Crohn’s or an IBD variant they have not identified yet? Biologics treat that too!
