Hello. I had my j-pouch surgery about 20 years ago.  In the past 5 years I find if I eat at normal speed it is possible to get very painful cramps. This year it almost always happens if I accidentally eat at normal speed and the cramps now last 24 hours. They feel like a punch in the stomach.    Drinking Metamucil helps. Could use any advice or information.

I am not sure what you mean by “normal speed.” That said, let me tell you of my experience. After 15-20 years with my j-pouch and only occasional bouts of pouchitis, I developed what I would have sworn was signs of bowel obstruction or an inflamed gallbladder: serious crampy abdominal pain after eating. It did not. Seem to matter what I was eating; even yogurt caused this pain. Turns out I had chronic pouchitis. Antibiotics worked, but I could not get off of them. That was when my GI recommended Remicade with Imuran. I have been in remission ever since.
