I had one prior to my j-pouch and it is now a source of much suffering. Before I learned to manage it I was brought to tears more than once. But my surgeon said trying to remove it would mean terrible pain and she recommended trying to live with it.
The best thing for me is to keep the area very clean and dry and avoid wiping as much as possible. Also apply a barrier every time you wash, but make sure you are dry first. Some people here swear by bidet seats, but I'm still making do with a squirt bottle of warm water. I also find sitz baths in warm water helpful. Then blot dry instead of wiping. When it gets really bad I use cortisone cream for a couple of days until it calms down again.
