Hello All:

I had UC, am 72 years old and almost two months out from my takedown.  I feel good and have been going to the gym almost every day.  I have been careful on what I have been eating, introducing fiber slowly but surely.  I am having anywhere from 4-5 bowel movements per 24 hour period.  The stool is not liquid and somewhat formed.  So I think most everything is on track

However I have experienced some problems and wanted to know if anyone out there has been through the same and have some suggestions.

  • Leakage at night. The leakage seems to come after I get a sharp shooting pain in my right side and then I feel it traveling through the intestines.  It is like I need to pass gas.  I take Metamucil in the morning and Imodium liquid before bed and that appears to slow things down. I wear Depends and place a piece of toilet tissue between my cheeks to catch small amounts of  leakage.  Not sure what else to do at this point.  But the pain and leakage occurs several times at night and keeps me awake.
  • Passing gas.  I still have problems passing gas especially if I am standing. However I have found that if I lie on my right side I can pass gas easily, but sometimes a small amount of stool will come out along with the gas.

I did not have these problems when I had went for my post-op exam and my next meeting with my doctor is April 10th. 

Any advice or recommendations would be appreciated.

Its completely normal to have leaks and hard time passing gas for 5-6 months post op., you're doing amazing for a new pouch. I had hard time passing gas for like 4 months and leaks upto 3 months post takedown. Dairy, eggs , potatoes ( not the sweet ones) , fried foods seem to be more gassy for me. But in moderation I can have them daily. Also sugar is killer for me. I cant have too much specially before I hit the bed. One chocolate bar and I'll end up having endless bathroom trips. As your output solidifies you'll soon pass the gas easily.

Hello Nick I also have same issue. I have leakage when I'm asleep. I have to wear a pad every night. And I go restroom every 3 to 4 hours. Also I developed indigestion problems have to deal with reflux and gastritis. ☹😪

I have had my j pouch for 6 years and very often have difficulty passing gas.  I NEVER attempt to pass gas without being seated on the toilet as I always pass a little bit of stool too.  Sometimes I bend forward (as if to touch my toes) and this encourages gas to travel from my intestine and into the pouch.  Spending a few moments like this often helps.  I have constant stool leakage but that has nothing to do with the pouch and everything to do with a seton drain I have  in for an abscess and fissure.  I love my pouch!

Nick, I've often wondered how the gas thing goes for other people.  Like Jay, I can't pass just gas standing or sitting.  It's a real nuance at work.

Here's my theory:  The colon absorbs water and the contents are get more solid as they get closer to the end.  We never absorb enough water for gas to slip by.  Gravity being what it is, our much more fluid contents settle to the bottom and the gas settles to the top.  Put the opening at the bottom and the gas has to go through the fluid to get out...

My poor wife.  When we go to bed, the opening is nearer the top and the gas has an open path...

Thanks all for your replies.  I am over two months out on my takedown and I am feeling much better.  However passing gas is still problematic, can't do it standing up or sitting, only lying on my side will gas pass through.  Additionally, there are times when small amounts of stool will pass with the gas.

I am hoping these two problems will resolve themselves in the future.
