Hi All,

I developed an abscess between my j-pouch and sacrum.  They drained it by going through my butt cheek and I have a pigtail drain in.  Then a few days later they put dye into the cavity and it shows there is a fistula - the abscess is connected to my j-pouch (no exit to outside my body - the connection is between the abscess and my pouch).

My colorectal surgeon hopes this will close on it's own, but to do so, there can't be poop going through that tract.  Her solution is to "defunction" me, that is give me back my stoma and allow this to heal.  I really don't want to go down that road again (I had my ileostomy reversal 4 months ago).  My surgeon says the intestine is too inflamed for her to stitch or clamp the fistula closed (it could do more damage).  I also suggested putting me on TPN so there was no food going through, but even then she said there would be a lot of fluid still produced by the bowel and it would not give the fistula a chance to heal.

Does anybody have any suggestions that do not involve another stoma? Glue perhaps?  I am having trouble finding info on glue used fistulas with no external exit.  They want to do the surgery in two days time.

Maria, my son has had the same thing for the last 2 years.  In January 2014 they discovered it.  Put in a drain, but him on TPN for 8'weeks.  Didn't work.  In March of 15 he went back to an Ostomy.  Shortly after that, he was diagnosed wi PSC so we had to change doctors.  We moved over to Weill Cornell in NYC.  New surgeon. He did lots of studies to see if the fistulia and abscess were going away.  He did another drain.  They did so much last summer.  In the end, by November it appeared to have gone away.  Did another takedown. In January 2016.  Well the darn abscess and fistulia are back.  In the exact same place.  Currently, my son has a drain in,Mia's been in since April I think, I don't remember.  The leak is has is so tiny they can't fix it without causing more problems.  So now we are thinking outside a box.  Last week he some studies done to be perhaps be fitted with a plug.  Like I said the leak is the size of the of a pin hole.  These things are extremely tricky and difficult to manage.  PM me if you want more details.  This entire situation has been extremely hard for my son.  He is sick of people poking at him.  You asked about the glue.  They did insert some about a month ago.  It generally doesn't work.  However, it seems to be working a bit.  There is less leakage coming out so perhaps it is working,  but to be honest, most fixes do not work for the long haul.

Maria, I don't know where you are being treated but I believe going back to the stoma is the best bet.  And give it at least 9 months to a year.  Do they know where your leak is?  That was part of our problem.  They knew it was there, but did t know where.  Turned out it was in the back of the small intestine at the sticking line.   Asamatosis?  That means the connection.  It was difficult to locate, took a long time.  I don't remember all the tests that were done..  But there were a lot.  Good luck, I understand how difficult this is.  And, no it's not easy.  If it's not one thing, it's another!
