Hey All, just going through a difficult time the past week, here's the run down. 

I have been battling with obstructions the past 2 years, and last year i was in emergency 4 times, each time the blockage resolved itself within 24 hours.

Two weeks ago i went into emergency again with an obstruction but this time it was slightly different as i developed a fever as well, and they gave me IV antibiotics. Once the blockage gave way i had very watery diarrhea with urgency.

When i came home i blended whatever i ate so it wouldn't cause another obstruction. I was still trying to get my bm's normalized as frequency was to high and then i get this feeling like I'm trying to empty the pouch but i feel constipated at the same time. I push a bit harder and then everything starts going haywire my sphincter muscles go into spasm theres a bit of blood on the tissue paper and i start freaking out. I sit down on the couch confused as i have never had this before and i start thinking that the antibiotics have just killed all my good bacteria and i cant get back on track and now things are out of control. The urgency is still present although this time i cant push anymore as the muscles are in constant spasm and cant relax. So i resort to liquid diet so everything will just flow without needing to push.

I called my dr and he said it could be an anal fissure and told me to get sheriproct and retrogesic. After a few days i started doing much better and although i had some urgency i could actually get out of the house. And then bam all of a sudden i get that constipated yet urgency feeling again and i cant push and empty my pouch. Anyway, went back to the dr and he wants to do a scope next week to see whats going on with the possible fissure and to see if he can find out whats causing the obstructions.

Yeh, so thats the story. Im still trying to figure out why i had a fever with my last obstruction? Could antibiotics and diarrhea cause an anal fissure? It feels inflamed could this be chrons? Im really nervous for the scope as the sphincter muscles are so tight how will he even get the scope up there without injuring me?

Ok, just needed to get this off my mind. If anyone can help me understand this it would be much appreciated.

Hey Wallflower, thanks for writing.

Just got scoped today, my head is kind of spinning. Dr found anal fissuring and dead end fistula at a shelf like narrowing just above the pouch that shows some ulceration.

 It was kind of assumed my blockages were due to adhesions but now the Dr thinks it could be Chrons due to the ulceration , he took biopsy and I have to get a follow up MRI.

Feeling a bit down, I have had my pouch since I was 13 and it's been 22 years of no meds and lots of fun. The last few years I have just not been feeling my strong self and I guess a Chrons diagnosis would explain it.

p.s. Just to add,  regarding the anal fissures rectogesic 3x day and mainly a protein diet have given me really good results.

Hey Drone3.  Your story sounds just like my year last year.  I was in emergency 4 times for obstructions...all which resolved within 24 hours to a few days.  Between x-rays, MRIs, scopes, and my history, there was disagreement between the GI docs on the cause (adhesions vs. Crohn's).  Then, a year ago, I ended up in the hospital again, but this time, it wasn't resolving.  After being my 5th time in the hospital, it was time to go in for surgery to remove adhesions and check things out.  The surgeons were able to gain a lot of knowledge finally being able to really see what was going  rather than speculating adhesions vs. Crohn's, etc.  At this point, after so many issues, you may be where you need to go in as well.  It's really a pretty definitive way to tell you if it's adhesions causing your issues or if it's Crohn's related.  Fortunately (or unfortunatley, however you look at it), they removed A LOT of adhesion bands.  The narrowing issue right near my pouch was also from an adhesion.  They ran through my entire bowel multiple times, and saw no evidence of Crohn's.  

While it's going to be a much tougher decision for you to choose elective surgery, I didn't really have a choice, but I'm very thankful to now know what's going on, and hopefully have several years of no scar tissue issues (even though I'm extremely prone to it and will likely have issues the rest of my life).

Thanks CLZ, that was really informative. I actually do think its adhesions and not chrons and i would opt for elective surgery but my surgeon is pretty much against it unless its an emergency.

My more immediate problem is fissuring and hemorrhoid, sphincter spasms and irritated anal canal. This is driving me crazy, i have had it under control four times but then it will just flame up and attack me with tenemus and crazy pain. I just made the stupid decision to go on a liquid diet and although it helped my hemrrhoid the acidic liquid BM's just burns and causes an inflamed canal with very sore sphincter. Moving back to my all protein diet with pro-biotics, rectogesic, sheriproct, paw paw ointment, witch hazel and hot showers. This has probably been my hardest challenge, with the the pain and it bringing back the dreaded memories of my UC when i was just a kid.
