its been a year since my take down and I'm still having pain after I go to the bathroom I'd say 8 out of 10 are painful. I just had blood work done since I moved states and my new doc said my white blood count was a little high could the pain be because there is a bacteria in my pouch? If it was pouchitis I wouldn't  be passing a whole lot of stool but I am just wondering if anybody else has had this problem before thanks in advance for any ideas

Primary treatment for fissures is sitz baths and control of the stool type causing them (straining at hard, dry, constipated stools or frequent diarrhea can both lead to fissures). Soluble fiber supplements or increased dietary sources can help With that. For deeper and/or more stubborn fissures prescription creams of nifedipine or nitroglycerine (old school) work by increasing blood flow to the area. Topical anesthetics help with the intense pain, but do not help heal. Some people are helped by oral muscle relaxants because of the severe spasms of the anal sphincters.

If it is on the outside, it is probably local irritation or excoriation from frequent stools, sort of like a bad diaper rash. For that you need a good barrier cream, and you may have a chronic fungal infection too. Baza cream is zinc oxide based and also contains an antifungal. Baby wipes are not good, as they dry the skin and can contain irritants. Plain water is better. A bidet is best for the best cleaning without rubbing the area.

 

Fissures are inside the anal canal.
