Hi there.

I have had UC for 13 years, and finally in December after a severe flare and having a bad reaction to infliximab/remicade I had the Stage 1 surgery (subtotal colectomy with mucous fistula, laproscopic)

It has been 4 weeks since the op and I am doing pretty good. I get a daily discharge from the rectum but its controlled so doesnt bother me for now.

My stoma output is pretty solid (well porridgey).

I am already starting to eat some veg and fruit with no problems, just a bit of extra gas.

Also I am doing light jogging and am easing back into work.

Although wearing a bag is not ideal, if I had to live with it permanently it wouldn't be a problem. I do have to empty it quite often, say 7/8 times during the day and once or twice at night, but then i do have a good appetite.

so my questions:

is the number of times you empty your stoma bag indicative of the number of times you go with a j pouch?

also i can have my last meal (dinner) at say 8pm, and my bag is still filling up at 9am, before i have had breakfast. Is a 13 hour total transit time common?

I have done plenty of reading on jpouch vs perm ileostomy.

I am still keeping an open mind to this but if I had to choose now I am leaning towards the j pouch just because I may regret it if i don't give it a try...

For me the number of times I emptied the bag was much, much more than I use the bathroom to empty my j pouch. I always had very liquid output from my loop ileo (I never had an end ileo which I hear is much better) and my j pouch my output is pretty much always firmed up. I use the bathroom on average 4-6x a day now with my j pouch. So for me it's been a night and day difference. I'm very happy with my j pouch. Best wishes with your decision.Happy to hear you are doing well. Smiler

thanks for your reply Marianne.

Is the difference in output between an end and loop ileostomy because of the design of the stoma or because you have lost more bowel with a loop one?

I am happy to give the jpouch a try, knowing I could always go back to an end ileostomy, but if that happened would my second end ileo be affected by the loss of bowel?

Hi Shainy

I eat my dinner by 6/630 pm and my ostomy bag still fills a bit upon wakening in the morning. When we go to sleep, our gut slows WAY down, and then it wakes up again when we do, and walking around in the morning, showering, eating breakfast all stimulate the gut to kick in. So, it's common that some people will have last night's dinner come out into the bag in the morning.

My ileostomy output was totally unpredictable but I always had to set my alarm to get up once or twice in the middle of the night to empty the bag or I would generally wake up in a terrible mess. Now that I have my j-pouch I don't need to wake in the middle of the night.
My j-pouch activity is also unpredictable. Sometimes I am in the bathroom every 45 minutes for a few hours and other times I can go 3-4 hours with out a trip to the loo. Touch wood I have never been incontinent with my j-pouch and am glad that I had the surgery. That being said, I am aware that one day my pouch might fail and I would have to have a permanent ileostomy. That is OK too.
