Hello all. I am having severe pain. I feel like I have UC again. I have severe pain when I have a BM. I am not screaming yet, but thats not far off. I feel like things are very swollen down there. It is dufficult to go the the bathroom. I saw the surgeon yesterday and he saw no sign of inflammation. He said to give it a week and see if it develops into something. That doesn't help me now. I am afraid to go to the bathroom because it hurts so much. I feel like I am pushing out a watermelon. If anybody has any advice I would really appreciate it.

Oh boy, this sure sounds like an anal fissure, and they can escape detection on exam, unless the doc is looking for it. Lots of posts here about them. People describe it to be like passing broken glass or razor blades. First line of treatment is hot sitz baths. Also, fiber supplements to add soft bulk and reduce the burning acid on the fissure. There are prescription ointments too.

Thank you so much for the reply. Anal fissure. I hadn't even thought about that. I was concerned with pouchitis. I am going to look it up. I have a low grade temp too. I am cooking thansgiving dinner for 16 people and I need to feel better. Do you think aloe vera gel would help? I haven't evrn been eating cuz I'm scared o.f thenk pain. It reminds me of when I .had colitis. Thank you so much

I wouldn't put anything on it except maybe a topical anesthetic, like Preparation H. If you have an aloe plant, fresh gel from a leaf might help, but all those commercial ones don't have active aloe vera gel in them, because it degrades quickly once it is removed from the plant.

If you think this might be what you have, you can call your doctor and maybe he'll call in a prescription for you to try.

potty, of course I defer to jan to nix/veto... my suggestion ... but I have good success with preparation H suppositories for a little extra soothing.... (as well as the prep H gel)... or if you can get a quick prescrip for canasa suppositories?? also, can you give food a rest for a day? just drink boost ?... see if things will calm down by little or zno traffic? good luck....

If it is a fissure, I don't think Canasa would help. But if it is cuffitis, it would. More likely that you would have bleeding if it were cuffitis. You get bleeding with fissures too, but not as much as with cuffitis.

Agree with the Prep H products. They help with the pain, and it is the pain that causes those terrible spasms of the sphincters that can last for hours.

Okay. I will try the prep H. I did try canasa as I still have a boatload, but it didn't help. I don't think that I can get cuffitis because my surgeon stripped the mucosa lining off of the cuff while he was in there to prevent cuffitis. Maybe he didn't get everything though. On my way to CVS. I will let you know what happens. And again, Thank you.

the one other obvious suggestion I can think of is.... are you sure it's not just super bad butt burn??
in which case definitely try ILEX if you haven't tried it.

You can order it online with Amazon.com. you apply it and it dries in a thin layer coating your skin. I have bad BB issues but if I use ILEX I'm good.

No its not BB. I had it really bad about three weeks ago. I am beginning to think that you were on to something when you mentioned cuffitis.
My symptoms are so similiar to what I went through with the UC. Temperature, throbbing when I stand, not being able to pee, and the I am going to go see my GI ASAP. He will listen to me. The surgeon just keeps saying everything is normal and doesn't want to listen. If things were normal I wouldn't feel like I was trying to poo a watermelon. Unfortunately,with Thanksgiving I won't be able to see him until next week. My goal is to stay out of the ER this weekend. Thank you again for the advice and letting me rant. People who haven't been thru it just can't understand.

the other suggestion I forgot about which I don't where I'd be without this.... Noxema cold cream. This won't help you prevent but after you are already in pain and just want relief a shower with noxema cold cream will get you back to ground zero. This was suggested to me by someone in this forum and it is a lifesaver for me. .... give it a try.... yes, use it "there" in the shower liberally....

If what you have are fissures, it can be very painful. I had the doctor prescribe me Lidocane ointment. It's a numbing ointment. Before I would go the bathroom, I would insert a small amount inside me and around the opening. It cut back on the pain alot. Sitz baths after were very helpful. Now, that being said. You need to eat. If you don't, all you end up passing is stomach acid which creates even more problems. Your entire bottom will be sore. Eat bland foods, bananas, cooked pasta, toast. Something so that when you do have to go, it's not pure acid. I've been doing this for 2 years.

Boy, I don't know about the Noxema if you have a fissure. That menthol can be pretty powerful on the fissure, like lemon juice on a paper cut! If it helps instead of making it worse, it probably is not a fissure.

Lidocaine ointment is great because it is a topical anesthetic , but is prescription only. So something to ask your doctor for, once you can get in touch with him.

Hope your Thanksgiving went OK. That was a lot of people to cook for with all that pain!

What you can also do is get a script from your doctor and ask for Nefedepine based cream. It is a mixture of petroleum jelly and nefedepine, which is applied to the butt and sphincter. It's purpose is to stop spasms of the sphincter, which will help ease the pain and allow it to heal.

This cream was a life saver for me when I had pain down there. I also used a barrier cream like calmoseptine. A combination of both helped.

Yeah Mike, that's the stuff! It contains camphor, menthol, and eucalyptus. Cooling on intact skin. It has some of the same ingredients as Vick's Vaporub. Your not supposed to use it on broken skin, that's why I was concerned if there was a fissure. But, it's fine on intact skin. It reduces pain by causing counter irritation (sort of like capsaisin).
