I have had my pouch since 1998 and have learned to live with its limitations. I am still going to the bathroom an average of 8 to 12 times a day. I will have to undergo a double mastectomy in the near future with reconstructive surgery done at the same time. I am almost more worried about how to get through the surgery and immediate aftermath with my pouch, then I am about the surgery itself. Any suggestions?

Can you say exactly what's worrying you? That would make it easier to advise/reassure.

One possible concern is pooping while under anesthesia. For a big procedure you'll have had nothing to eat, so there shouldn't be much coming out. I did have a *small* mess (a big shart?) during an upper GI endoscopy, so I'd prep for that (no food, maybe different clothing/barrier choices) if I had to have it again.
I had an "open" hysterectomy about two years ago and although my surgeon wanted to do it lapro....he couldn't because of adhesions from the two J pouch surgeries I had had. No problem. Same scar line as colectomy scar. Very, very easy recoup and J pouch performed like a pro! I was well cleaned out prior to the surgery so no leaking, etc. I expected the hysterectomy to be as serious a surgery as the colectomy was. No way! Best wishes to you!
I've had several surgeries without issue. I do have weak sphincters/incontinence, which is exacerbated when my system is "empty" so I let them know that when I am in pre-op. I usually compensate by tucking a cotton ball up there and keeping underwear and a pad on where normally they might have a person sans undies in pre-op, until they absolutely must take them off (like to put a cath in or something). If there is a little leaking or mess, that is what those blue chuks are for Smiler I just let them know the possibilities so there are no surprises.

Sorry you have to go through this, but I don't think your pouch should cause you any issues.
