Hello,
I am writing to see if anyone on this site can relate to what my father is going through. In short, he had a J-pouch for 5 years which worked great until he had pain with a hole in the pouch. He had to have a new pouch reconstructed and he is about 6 months out from the takedown. He currently has NO control of his bowels! He is a prisoner to the house and can barely get up to walk without soiling himself to the point of needing a full shower. He has lost 20lbs in 6 months and has butt burn so bad that he is in tears just trying to walk. His doctors here in Philadelphia are baffled and really don’t have any answers for him as all his scans look great. I made an appointment for him to travel to Cleveland to see Dr. Bo Shen on March 4th. At this point the trip there is going to be a challenge because the plane ride could be disastrous. I have read many posts on this site and I do see a lot of people complaining of nighttime incontinence but I never see total incontinence, day and night. If Dr. Shen can’t help him he needs the bag back fast before he is too thin to undergo major surgery. Has anyone ever experienced this?? Any advice, help, or encouragement anyone can give is greatly appreciated!

Since it appears that his pouch seems fine, it sounds like he suffered sphincter damage or nerve damage during the surgery. After more than 6 months since the original surgery, things should be showing some signs of improvement. It could be from trauma to the nerves during the actual reconstruction or the prolonged anal dilation for reattachment.

Without a colon or rectum, it takes less sphincter weakness to cause incontinence. Age could also be a factor (our sphincters naturally weaken with age).

Before the long trip to Cleveland, I would suggest he have rectal sphincter testing done (manometry, ultrasound, EMG, defecogram, etc.) to test the strength and function of these muscles, if time permits. The results might give some clues regarding what is going on. Then, when you go to Cleveland, you are armed with more information.

Hi,

I do not have any information to help with regards to incontinence, but perhaps I can help with some suggestions for alleviating butt burn.

There is a compound of nefedepine cream and petroleum jelly that can possibly help with the butt burn. I would also recommend using calmospetine as a barrier to protect the skin.

These two suggestions can hopefully make a difference for the pain.

Thanks for the feedback. My father is at the Cleveland Clinic now and had the knife procedure by Dr. Shen yesterday.Dr Shen found a small sinus and also balloon dilated the opening of the pouch. Something must have happened during the night because he was in excruciating pain. First he thought the pain was from being shaved because his butt cheeks were red raw but the pain was like being burned with a torch he said. It got worse and seemed to be in the tailbone area. Dr. Shen just admitted him to the hospital today. My parents were due to come home to Philly today but now they are there. At this point my dad wants the surgeon to pull the stoma out for some relief of the pain and nonstop diarrhea. I don't know if they will do such a thing while he is there. At this point I hope they do.

This site is great! I read some similar stories and some are so hopeful!
