Hi everyone,
So I've battling pouchitis since last August. Was put on flagyl and seemed to be doing ok. But 2 weeks after the completion of the flagyl dose the symptoms reoccurred soooo I was put back on flagyl again. I've been on 200mg every second day for now about 4-5 weeks and have started getting the nausea, abdo discomfort but no diarrhoea. So I guess the flagyl is not working for me. I have an appointment on Friday and really want to push to be placed on something else. Just wanted to get some advice of other possibilities of medications from others. By the way iam from Australia so hopefully anything suggested we will have here

Cipro often works astonishingly quickly on pouchitis - within a day or two. Most people do fine on it, but like most drugs it has a long list of possible side effects. I was on it for several years without a problem before it stopped working. One of the concerns with Cipro is an increased susceptibility to C Diff.

Cipto works very quickly...within days. It thickens the stool and slows the output. I often get muscle aches when on it as I was on it frequently for over two years. I also had a hard time sleeping, but not sure it was related to the medication. The other real concern is tendon rupture which is rare but a possibility, and antibiotic resistance if on it for extended period of time.

I also think Cipro works great and within hours, however, I wouldn't consider the Flagyl not working for you at this point. Flagyl is typically supposed to be take 3 times a day. Taking just one tablet every other day is not usually very effective for an antibiotic. Additionally, if you are thinking you are antibiotic dependent, it is time for you to alternate to a new one anyway. Staying on the Flagyl too long will cause it to lose effectiveness in the long run.

I've been on Cipro for 10 years. I had the same thing as you, I'd try to get off it only to have it come back in two weeks. I take 500 mg daily as a maintenance dose. When that doesn't work, I double the dose for two or three weeks, then taper back down to once a day again. I also take VSL 3 DS daily, but stop it when I'm on double Cipro because I figure the double dose of antibiotics will just kill the probiotics off too fast. I had a new nurse once try to tell me VSL wouldn't work with even the once a day Cipro. I almost laughed and nicely told her that after 10 years of this, I knew what worked for me and what didn't.

I was on cipro and flagyl myself and not getting much results on top of having tons of side effects that made me feel even worse. I'm being seen by the infectious deseases because of other issues... And well he switched to avelox and it's been almost a week and I feel better the nausea and muscle aches are way better. As for the pouchitis it's better but I'm still experiencing with leakage Frowner

When my chronic pouchitis started, I started having bad side effects from flagyl and cipro didn't fully treat. I now rotate levoquin and Rifaxin for antibiotics, and also use entocort and Immuran (because rotating antibiotics alone does not work for me, unfortunatly). I have heard augmentin is another good antibiotic to try. Lots of options out there-don't give up!

What dosage of cipro and or flagyl is safe to be used daily long term?

When I have bad pouchitis flare up, I take 500mg cipro twice a day with 400mg flagyl 3 times a day for about a week. Then I just take 250mg cipro daily as a maintenance dose. Been doing this for two years. Is 250mg too weak? 500mg better and safe to use long term?

What is you daily dosage guys (and dolls) ? Smiler

I have been on 500mg daily of cipro for about 12 years, it has controlled the pouchitis but the side effects are really bad, I tried to quit taking it and 4 days without it my insides were very swollen and sore, I went back on the cipro and the next day was fine, I'm worried the cipro has caused permanent damage, has anyone else experienced this? My side effects from taking it is tendinitis, Achilles tendinitis, I'm losing the strength in both of my hands and they stay numb and tingle, I had a retinal tear and I'm having hearing loss.

You are aware that rupture of the Achilles tendon is one of the side effects of Cipro, correct? You MUST get on a different antibiotic if you are antibiotic dependent. If your current doctor will not change the Rx, you need a new doctor. I cannot stress this enough! In your other post, I see that you are seeing someone new, but you really should stop the Cipro now or reduce the dose or frequency.

Try reducing the dose by half. That may be enough to keep the pouchitis symptoms at bay and also reduce the side effects. The fact that you are very active is even more of a reason to take this side effect seriously. A ruptured Achilles tendon will interfere with your work much longer than taking a few days off now to deal with diarrhea that may occur with a dose reduction of your Cipro.
