Sounds like a pinched or irritated nerve. Is this a new symptom, or has it been occuring since surgery? If it has been going on since surgery, it could be related to scar tissue or swelling that can improve with time. If this a new symptom, you definitely need to report it to your doctor. Since it is not constant, it may be difficult to diagnose, but still should be investigated, if only to rule out serious stuff.

Yeah, when they build the j-pouch, there can be bleeding/swelling in the lower pelvis. This can take a long time to simmer down. Plus, scar tissue can get bound up with the nerves down there. It can seem to be worsening now if you are increasing your activity level, even if things really are stable or improving. The fact that this is positional supports this.

Never the less, you should call your doc and let him know about the symptoms and let him decide if you should take the wait and see approach, or come in for an exam. Hopefully this is just a little bump in your recovery road. Sort of a sign that you might be overdoing it a little.

Pain is always a sign that something other than normal. But, expected pain or explained pain that we anticipate to resolve with time is different than pain that is a sign of a complication.
