Hi everyone, I am about 4 months out from my ileostomy reversal. I had pouchitis symptoms which improved with cipro and then came back once I stopped it. Pouchoscopy then confirmed pouchitis so I started a longer course of cipro which has been helping. It also unexpectedly showed cuffitis and because I developed rectal pain/spasms and increased frequency I started Canasa which helped for a short time. Despite being on the Canasa for a few weeks now- a med that worked very well for the proctitis part of my UC- I still have rectal pain and frequency. I don't have a stricture or fissure but I am using nifedipine/lidocaine for the spasms. My diet hasn't changed and I'm using imodium/lomotil/fiber/pepto as needed.

So, what is causing my pain, spasms and frequency? Is this cuffitis not responding to Canasa? Pouchitis no longer responding to Cipro? Something else entirely? Anyone have any thoughts?

Sounds exactly like what I have been battling for months. I have chronic cuffitis that is pretty resistant to treatment with canasa and hydrocortisone suppositories. I also have a fissure and use nifedipine daily.

Both pouchitis and cuffitis can increase your frequency and cause discomfort. Sometimes if you have been on one antibiotic for a long period, you may need to switch to another one so it does not lose it's effectiveness.

This often takes time to sort out. It can take several weeks of treatment for cuffitis to get relief and this applies to pouchitis also. Maybe you can also try some bowel slowers to help with the frequency. Hope you feel better.
