i have this pain on the right side of my anus right where i have a hemorrhoidthats subsiding but doc says it shouldnt still hurt so he wants to scope. 6 weeks is earliest he can get me in but sent me home with hydrocortisone suppositories. he mentioned he wants to rule out pouchitis cuffitis and fissure. what would you do? it seems to me if docs suspect these things antibiotics are in order quickly not wait 6 weeks to scope. could it just be the remnants of the fading hemorrhoid or perhaps and unfounded internal hem? what are hydrocortisone suppositories good for? this pain is constant and i might try them to get relief...

There seems to be a fair amount of "anal" pain being reported lately. Kind of strange since I'm one of the people reporting it. I'm sorry you are going through this.
Your pain could be a result of many things... Hemorrhoid, fissure, pouch/cuffitis, or none of the above. Without the scope, you can't know for sure.
It has been my experience that some sort of steroidal foam, cream or suppository is usually prescribed for this sort of pain, probably due to the hope it would relieve inflammation, etc.
Are you having any other symptoms? Sometimes Dr's are Leary to jump right to antibiotics because of their own set of possible side effects and complications.
My advice would be to call the office on Monday if it gets worse or you are simply very concerned. We have to advocate for ourselves sometimes
I'm near 8 years post takedown and had 6 perfect years with no issues at all (didn't even have a surgeon or GI locally when I moved to Florida) until I started with stricturing and cuffitis and have been struggling for 2 years now. i'm not saying thats whats going on with you obviously, but the sooner you find out what's going on, the better!

P.S. There are many different types of creams to use for various types of problems... I've probably had them all, so I'd be glad to help in any way!

The only diagnosis you mention that antibiotics would be prescribed for is pouchitis. He could rule out/in cuffitis and fissure without scoping, so a simple, short office visit and digital/visual exam would suffice, without scheduling the endoscopy clinic or OR for a scope. He should be able to squeeze you in under shorter notice than 6 weeks. If I was as miserable as you, I would even opt for someone on his team or even your GI, to be seen sooner.
I've been prescribed Flagyl for presumed pouchitis without a scope. The scope came later when a single course did not suffice. Cuffitis usually includes the symptom of bleeding, although it does not have to be serious bleeding. With me, it was pretty minimal. It is easily diagnosed by a digital exam, as the rectal cuff feels "bumpy" when it is inflamed.
The fissure could also be seen without a scope and you could well have one at the site of your hemorrhoid. HC suppositories might help that, along with nifedipine ointment. You can also get lidocaine topical gel for the pain.
I'd be bugging him for an urgent appointment.

surprisingly he called me upset with the schedule and moved me to this tuesday. it is a small hard lump that was hidden by my hemorrhoid. just talked to his nurse and told me it likely not an abscess or fistula because of how long the pain has been substantial (4.5 months) and the fact that it doesnt hurt to the touch. im going to try to make it to tuesday without paying too much attention too it and freaking myself out. thanks guys so much...
Yeah, until he actually looks at it with his beady eyes, it is all conjecture (OK, I don't know if his eyes are beady or not, trying to make a funny here ). Yes, a perianal abscess should be painful to touch. Maybe the lump is from the hemorrhoid and there is a deeper, perirectal abscess. Can't say for sure. But, this is definitely in or around your anus, where there are a million nerve endings (or at least it must seem so)...
