A defogram probably would not show a leak, unless it was huge. The contrast is sort of a thick material, so would not enter a tiny leak. It s for visualizing the actual pouch funtion/motion during a bowel movement.

The MRI is best suited for identifying leaks because it visualizes the soft tissues in cross sections.

The principle objective of the MRI Enterography is to visualize if there are areas of thickness in the small bowel wall that are indicative of inflammation.

thank you. Our GI just said a pouchogram is the same as a defogram but in a defogram I thought you had to simulate being on a toilet by sitting so would that make a difference?

No, it is NOT the same as a pouchogram! The pouchogram is static and only shows a resting, distended pouch. The defogram shows the actual motion of the pouch during defecation. So, it is the ONLY test that would show if there is colapse or other dysfunction that is not visible on scope or pouchogram.

A defecogram is a completely different test than a pouchogram. I mentioned this to you in another post a few days ago I believe. I believe he needs the defecogram to understand if he has "mechanical" problems emptying whether due to efferent limb, twisted pouch, pouch too big, or other things. In the defecogram, they fill you up with some kind of contrast, I don't remember what or how, probably it was enemead up the backside. Then you sit on a "throne" and try to poop. This is all visualized (radiographically) on a monitor right in front of you.

Also an MRI may be needed to look for sinuses at the anastamosis line which may be what is causing the bad cuffitis he has.

My GI I had after takedown never ordered the defecogram either despite all the problems I was having from day one of takedown. But when I got to CC fivce months later, immediately after Dr. Shen scoped me, in fact during Dr. Shen's scope, he ordered an MRI and a defecogram, and a surgical consult.

I think it is time for you to get a new GI doctor. Your little boy sounds very sick and I think you and he both deserve a better GI doc if you can get him to one.

Any way you can take a few day trip to Cleveland Clinic to see the experts and get your little boy some help?
