I am just curious what the difference is between the 2 kinds of pouches and why some have the k and some the J.

The j-pouch is an ileoanal pelvic pouch. The k-pouch is an abdominal ileal pouch with a continent ileostomy-- no external appliance.

The k-pouch was developed before the j-pouch and once was the darling of UC surgeries. However, the j-pouch is simpler and with fewer complications, so it has virtually replaced the k-pouch.

The k-pouch is done (where available) when the j-pouch is either impossible to do due to structural issues, or for a failed j-pouch as an alternative to an end ileostomy.
