I was wondering if a diseased jpouch, severe crhonic pouchitis, with no hope of getting better could be replaced with a new jpouch all together?Instead of ending up with an ileo would it be possible to try for a second pouch and if so would the second pouch suffer the same thing as the original.I hope I haven`t confused the hell out of everyone, lol.

My reading of this is that you are wondering whether your entire
existing pouch could be removed and a new one created from what remains of your small intestine. You could ask a surgeon to verify this, but I would be surprised if that were possible, since it can be difficult enough to free up a sufficient length of small intestine to make the first pouch: http://www.ncbi.nlm.nih.gov/pubmed/17824985?ordinalpos=...anel.Pubmed_RVDocSum Plus, there is every reason to think that the second pouch would soon suffer the same fate as the first one. I expect most doctors with whom you might consult would recommend first trying to save your existing pouch by clearing up the pouchitis. That could involve dilation of any stricture, antibiotics, probiotics, and other medications commonly prescribed for pouchitis. After patiently trying all that, if I were in your position, I would opt for a straight ileostomy. No way would I go through another major surgery that was not likely to improve my condition. You can find many posts by people on this site who have made the decision to go back to the bag, got their life back, and have no regrets. I do admire your "true grit" in trying to get the results you want without taking yet more medications or needing a bag! This message has been edited. Last edited by: bobolink713, |

Hi Anna,
Yes there is enough small intestine to make a 2nd pouch. Please read this thread, where a 2nd pouch for pouchitis was discussed: http://j-pouch.org/eve/forums/a/tpc/f/5951071921/m/4411...411015832#4411015832 I wish you all the best, Nikki. |

It is possible Anna, but I would think it might be an individual thing. There may be certain criteria that needs to be met. I had some problems and I spoke with my surgeon about the possibility of a pouch redo, I didn't need it, but the option was there for me. Good Luck,

Thanks for all the input.Bobolink makes a goodpoint about having another major surgery as far as trying for a second pouch but the way I`m living now any alternative would be better including another major surgery. In trying the medication route how long would you figure it will take for me to see a difference in how I feel taking both cipro and flagyl. Also would it be true that once I start with these 2 meds I have to stay on them for ever or at the least a very long time? I`m already on some pretty potent meds for another issue also dealing with my surgeries.Thanks.
