Hi

I have a fistula which needs to be treated by a seton. Does anyone know of a really good doctor based in Sydney Australia?

Also, my seton was inserted 5 wks ago and all was going fine until this week where it appears the abscess is reforming indicating infection is building up. Does this mean that the seton is no longer working or that another opening into the rectum has formed?

Thanks for any help.

hi tt, i lived in sydney aust for 16 years and came back home over a year ago for my operations, had TD 5th nov. when i lived in syd i had great GI and surgeon (who was trying to convince me to have the op) the GI was Warwick Selby and the surgeon was Michael Solomon, they are both at RPA in Camperdown. i obviously did not have the surgery with him but he was very kind, knowledgeable, had done lots of these ops and came highly recommended. Goodness...why are you having a seton??This is an outdated op that doesnt work well for pouchs. I am the Panpan mentioned by the lady above. Unfortunately, my email got hacked so I had to make another profile. I have two fistulae. I had one operated on in the US..operation was a failure. I have had several opinions on them in the US and Oz. What investigations have the docs done of yours? You should have had a pelvic ultrasound and a pelvic MRI to see what is going on. Then, and only then, can a doc give you the right diagnosis about how to fix the fistula. The accepted and successful methods used for pouch people are: advancement flap with fibrin glue; porcine plug and stem cell treatment. Disaster treatments include: fibrin glue only, fisculectomy, and loose/tight setons. If you can tell me more, I can recommend someone who can treat your fistula effectively.

In Oct 07 developed an anal abscess, went to hospital where it was drained, told have anal fistula but no communication with rectum.

1.5 wks later swelling/inflection again, go to a different surgeon who uses dye to check for entry in rectum, told all ok, abscess site carved out to 'heal from the inside out'.

1 wk later swlling/inflection again, this time finds opening into rectum. Tells me only treatment option is cutting seton which is inserted Nov 07. Check-up in Dec and told all is fine but last week noticed swelling/inflection again.

Have not had any tests and don't have a pouch and I'm worried that another fistula is forming. Would be very grateful for any advice on this.

BTW: told by both surgeons and my GP that the abscess formed due to an inflected internal gland.

Hi all

I also have a fistula (as a result of an abcess) and it has been treated with setons over the past 18 months. I'm interested to learn about the evidence for this not being an effective treatment as it seems to be standard treatment here in the UK.

Bowella, has your fistula been treated successfully by any of the treatments you mentioned?

My surgeon is only suggesting 2 options now Infliximab, or pouch removal. I'm very worried about both options and am continuing to put up with the horrible leakage while trying to decide what to do. Both options seem very extreme to me - like using a sledgehammer to crack a walnut! Although I do have intermittent pouchitis/cuffitis flares I am generally fairly well. My other main problem is most pouchitis antibiotics cause significant joint problems so my options for treating flare ups are gradually diminishing.

Grateful for any info from those who've had successful fistula treatments.

I also had a fistula after an abcess (which was the result of a sphinceroptomy after dreadfull pains with my anal fissure). I was operated on and had the abscess removed, and a setton stitch was inserted as I had a "high anal" fistula. Ive now had a Cooks plug inserted in the hope that it will be cured (1 week ago), so hopefully it sbehind me, but Im still in pain and the plug is oozing still

I also have a a perianal-pouch fistula and have a seton in place. I am also curious about it not being effective. My last MRI show that there is no longer a open fistula track but the seton is still in place.
As far a a doc in Sydney - As mentioned above:I would call the Cleveland Clinic they should be to assist you..
