I'm not sure why you would have to mention it at all if you are physically able to perform the job duties with the ileo. The only reason you might want to mention it is if you are planning on have a pouch created or TD in the near future and you'll need the time off for surgery/recovery.

No need to mention it, and, in the US, employers can't ask about health history. What they can ask is if you can perform the job, with or without accommodations.
There are few jobs I can think of where an ileo would prevent you from doing as good a job as any one else, provided you are well recovered from your surgeries and the ileo is functioning normally.
