Trajectory...big problem is finding your trajectory...not all of our valves are strait so some of us need to direct the tube in a certain direction especially when full...I have finally figured mine out...my pouch gets squished up when it is full and my bladder does too then I can't get the tube in so first I need to fully empty my bladder, sometimes 2 or 3xs so that the tension goes down a bit. My valve when pushed faces north so I have to push the valve upward to go 'over the hump' sort of...it works for me but maybe yours squishes left or right or is mobile...in which case the direction may change...
Try (when your pouch is reasonably empty or just half full) to find your trajectory and then follow it...you may discover that it always heads the same way...if not you should avoid allowing your pouch to overfill to avoid damage to your valve.

It could be worse...oh, wait..it already has been! then I guess it can only get better from here....

are you using lots of lube? I know that lube will help it guide along. And, sharon is right about it may need to gently twist or turn -but never force and if it's pinching stop and relax and reattempt. I agree not to allow it to get to full if this is a problem. Or at least interbate to get gas out only if in a hurry and keep it less expanded.
