So I am sure if you have read any of my past posts you would know I have been dealing with constant pain and now incontience with my J pouch. My pouch is almost 1 years old and with my last pouchoscopy last week the docs found a little pouchitis and an otherwise 'beautiful' pouch. None of the docs can explain this incontience problem. Like last night I just squatted down to talk to my 2 year old son and I lost control! At night I have constant nightmares that are trying to wake me up only to wake up and find a huge mess...I am pretty tired of all of these problems and have been weighing my options.

I am scheduled for a sphincter repair next month but am not convinced that that will help. Jan mentioned that if you have had a transanal surgery the sphincter might be damaged, well, with my last surgery Last July to fix the twist in the pouch it was all done through my bottom!(I was supposed to wake up to a temp ileo but instead I had a rectal drain for 4 days)

So, I am interested in knowing more experiences of people and their K pouches. Pros and Cons, likes and loves, even the 'I wish I hadn't's.

All the details that you want to share would be awesome. I really would love to hear from those who had a J and now have a K...

I had my j-pouch for alittle over 1 year. It was the biggest mistake I ever made. I had to be dilated so many times and had to do it myself, it never worked. I also had a couple of minor surgeries to fix the problem, but nothing ever seemed to work. My surgeon finally threw his hands up and said he could do no more and wanted me to go to cc. I started checking on k and BCIR pouches. I decided to go to Dr. Schiller in LA. He tried to use my j-pouch, but it failed 3 months later. He thought the next step would be to make a new pouch and valve and start fresh. I am so glad I listend to him. For me I couldn't be happier. I sleep through the night and empty 3-4 times a day, I eat almost anything now. I'm 5 months out from last surgery and I would do it as many times as I had to again.

I had chronic pain in my j-pouch for a year, even though it was never connected. I would choose a koch pouch over and over again. I love it! I empty 2-4 times a day and always sleep through the night. Its not without its challenges and risks but for me it has been well worth it!


July 21, 2007 - Subtotal Colectomy w/ end Iliostomy-----

November 30, 2007 - Proctolectomy with ilial-pouch-anal anastomosis and temp loop ilio----

November 11, 2008 - Failed j-pouch used to form a koch pouch

What is a BCIR pouch?
Also I've heard a few good things about a k pouch and a lot of bad things.Thinking about getting one because leaking bad with a j pouch.THANKS

A BCIR is a modified kock pouch that Dr. Barnett created. It has a living collar valve. They are both made with an internal reservoir and a flat stoma that you use a catheter to interbate (insert) to empty poo.

They would basically have the same issues and that mostly surrounds a diet that must be chewed very well to enable poo to flow through the small holes of the cath. If a large unchewed peice gets stuck, you take a peice of tissue and manually pull them out of the holes of the cath, the reinsert and empty further.

Both valves have the same 10 - 15% failure rate. The key is to have a qualified surgeon perform the surgery.

I had my k pouch done 30 years ago due to full fledged anal incontenence..I had 1/2 dzn anal repairs but none ever worked..strictures, snaping of gracilis muscle flap etc...the k pouch stopped the nightmare and gave me my life back...
I have had problems recently with the valve and repairs but the bottom line is that when it works right..it is heaven..I am as good as if not better than anybody else..can go and do anything and can eat almost whatever I wish..if I just chew...but you need a very competent surgeon to do it and to follow you up....

I am so glad I found this site. I live in Australia and there is nothing I can find on it. I had a J pouch 17 years ago due to UC, it been mostly fine, but just lately its been over-active, 7 x an hour, lost a few kilos, which is good Smilerbut I hate it now and I would love to go to a K-pouch. The last time I saw a specialist(my original creator has retired)I was told if I don't want to keep the J then I have to have an external appliance, which I hated too. I read on the net about the Kock pouch(BCIR)and wonder why this was never mentioned. I can't find anybody who does it here, but when I see the specialist next week I'm gonna ask about it, so wish me luck!!!
