prolapsed ileostomy

I had a loop ileostomy put in back in June 09 because my jpouch had never been great and I had a perianal fistula form and that abscessed. It prolapsed when I was moving my keyboard and amp, I'm a professional pianist, and I went to the ER of the local hospital, which put me in an ambulance and transferred me to a bigger hospital. The surgeon created an end ileostomy which has been doing fine, but it prolapsed last week after shoveling snow. Unlike last time, the color hasn't changed and I can get it to go back in after lying down for a while. I am told that as long as it remains pink, I have to learn to live with it. Anyone else in this situation?

Sorry, my hubby gets the opposite.. inverted stoma. But, I found this.. if it helps any:
http://www.allaboutbowelsurger...inical/prolapsed.htm
--there are a couple of options for what might help with a prolapsed stoma there, but not much. One suggestion is to use an extra flange adhesion for the extra weight of the prolapse. They suggest Welland Medical HydroFrame flange extenders. The othere is to use a very large bag.
There's this, too --may be much more helpful:
http://www.wellsphere.com/dige...y-nbsp-videos/862104
http://www.youtube.com/watch?v=wUs7Jn6J5kg
Other info I found said to use a "flat and flexible pouching system".
All of this is just what I found online.. I have no experience with a prolapsed stoma, just an inverted one, sorry.
Good luck!

HA! This is EXACTLY what I have. Mine suddenly got much larger, telescoping out. I don't think anything changed or suddenly happened. When I sneeze or cough, I feel like it's pushing out, I'm sure it just feels that way. Some days it's really large, and some days it's only slightly larger than it was originally. At the first of the month it was back to normal post surgery size, that lasted 1 day. I had my End Ileo permanant Ostomy created Oct 2008. I've had no problems with the larger stoma, all looks perfectly normal. My wife freaks out everytime I change and demands I call the surgeon. I'm just gonna let it ride.

I know that it isn't exactly the same but I have a K pouch (a continent stoma/valve) that herniated/prolapsed 4 times...the first time I had no idea what a prolapse was or what to do about it...but I hernaited due to starting to workout way too soon post-op...each repair was due to my not being conscious of how fragile our abdomens get after all the abuse they take...In my case the last repair was done with a mesh reinforcement through laparoscopy...and I have been warned that even that will not withstand moving furniture, shoveling or carrying 4yr old grandkids...be careful...even though we all hate to admit it..we are move fragile than we used to be..
Be nice to your stoma..and it will be nice to you Wink

It could be worse...oh, wait..it already has been! then I guess it can only get better from here....

I thought I was really careful, how long does it take? Or is this a forever fragile thing?

I wish I knew...it seems that once you have started having problems that you are more prone to have more...I am doing little bits of yoga and pilates but it is a real far cry from powerlifting and running stairs etc...am feeling rather handicapped here with all of the 'sorry but I can't do that any more' stuff...I don't know if the mess repair is considered 'healed' after a certain time or not...Especially when it concerns the area around a stoma...if you find and answer..let me know...I am dying to go back to being me...(painting my house, carrying groceries miles up/down hill etc)

My gosh Meghan. The video of that prolapse scared me to death! He had a good foot of intestine there, and acted as if "It is, what it is". Is there nothing that can be done to get it back to normal?? How does that happen, and can it happen with an end Illeo ?

Mike, I think that hernias can happen to anyone but especially to people like us with extra stress on our abs...here they talk about 2 types...hernia and 'invagination'...the hernia is when a 'normal' person lifts or pushes something too heavy and 'seperates' a muscle group that allows their insides to peek out during any effort (think coughing or straining'( I kind of like to think that it ressembles a run in a stocking Big Grin) ...an 'invagination' is when the hernia is on the site of a pervious surgery...like an old stoma site that was left to close on its own or a scar...
You are much weaker at your old stoma sites or especially at your new stoma site (it is a hole in the muscle any way that you look at it!)so it makes us more prone to hernias...
So you can keep lifting that baby in your arms until it reaches about the age of 4 Big Grin

okay I'm curious about the videos but since I have an end ileo it sounds like they might freak me out more than a little if it had people on this board weirding out.

I just returned from the ER tonight for a prolapsed ileostomy. I have a perm loop ileo, and this afternoon I went to Costco with my husband and daughter. I didn't think I lifted anything too heavy, but later in the evening, I noticed that my stoma was bulging. I removed my bag to take a look and almost passed out. I had a good 6 inches sticking out, and the stoma was super swollen I drove myself to the ER, (since my 2 year old daughter was asleep, so my husband stayed home). I was freaking out and crying. I was standing up, and showed the nurse. It was looking even worse and more swollen. The nurse had me lay down and wait for the doctor to come it. About 15 minutes later the doctor came in. I had been laying on my back waiting for the doctor. When I removed the bag to have the doctor take look, the stoma had gone down and everything looked normal. I felt silly being so upset when all I had to do was lay on my back. I was discharged from the ER, and now I am home. Since this happened to me once, does it mean it might happen more frequently, or does this just sometimes happen?

Hi,

Just thought I'd add to this - when I had my end ileo I got prolapses all the time! I could go a month or so without, and then, wham, I'd get them repeatedly. I had a lot of intestine come out too - about 6 inches at a time. The first time it happened I freaked out and went to the ER, but by the umpteenth time it became no big deal (the things you are learn to live with are astounding!).

Usually, just reclining on my back for a bit and pushing gently would get the whole thing back inside where it belonged. The doctors said I only had to worry if it didn't go back inside as then you risk the bowel dying for lack of blood supply and it is serious.

I was told that if I was going to keep the ileo that they would perform a simple surgery to "tack" the intestine down on the inside so it would stop prolapsing. Fortunately, I was able to get my jpouch created and reversal done sooner rather than later, so I never had to have the "tack" surgery done. Might be worth exploring if you have a perm ileo and prolapsing is a problem.

Prolapsing was a big annoyance for me because it tended to prolapse whenever I exercised or leaned forward for long periods of time (e.g., working at my computer), both of which I do a lot of! It wasn't always feasible to go excuse myself to lie down - especially at work.
