My jpouch is 18 years old...in that time, the only time I would have incontinence or leakage was during severe pouchitis...it would happen while I was sleeping as my pouchitis usually feels like gas is pushing the stool down into my pouch...actual leakage over the 18 years was quite rare for me...in the past 1 1/2 months I had a several things occur at once...the first being sacral and leg pain, the second being pain above my rectum and below my tailbone (had mri and it was negative for an abscess or fistula) and the third is pouchitis with incontinence/leakage while I sleep...now I must say that I have a history of lower back issues with bulging discs (seeing a PT for this)...in regards to the pain in my buttocks...not sure what to do here as the mri was negative but it continues to bother me when I sit and when I go to the bathroom...and the incontinence is really bothering me...my pouchitis is better but not fully gone and I will talk to my doc about this...I recently started cipro after not using it for a year and its not helping like it use to...I just started alinia again and recently used tindamax...all of these drugs help me but somehow nothing is banging it out of me like it used to...I also take humira for my stricture above my pouch and I take xifaxin...not sure if that is much help anymore...anyway...I was wondering if anyone had any ideas what to do about the incontinence at night...I'm not sleeping well for fear of having an accident...also, my GI stumped as to what my pain in my rectum is...any ideas?...debbie

I have similar experiences to yours. My jpouch is 17 year old, and I too have started experiencing night-time incontinence in the last 2-3 years. I've had pain in the sacral area (which also pains my butt, hip and down the leg at times) off and on for many years, which was diagnosed by a rheumatologist as sacroiliitis - I don't think is uncommon among those of us with IBD. However, I would think if you had that it would have shown in an MRI?

I'd like ideas on night-time incontinence, also. I realize that pouchitis can contribute to this, and I was recently treated for that with levaquin (I have problems with cipro and flagyl). Following that, I'm trying 2 pepto-bismol/day, and 2 doses of probiotics/day. Finally, thinking this could also be age related, I have started to get a little more serious about doing kegel exercises. I'm doing a little bit better with the incontinence, at least for the time being, with this regimen.

Last but not least, using Kotex maxi-thin pads at night, along with barrier creams, gives me a somewhat "confident" feeling, in that if I do have an accident, I won't create a mess and my skin will be protected - I feel that I can sleep through the accident for awhile, and there won't be a huge problem with it. I hope this helps a little.

I have similar issues but was diagnosed with pudendal neuralgia.

It feels like a nerve is being pinched and making me have pains in the groin area and down the inside of my thigh near the joint.

Also makes me feel pressure-pains under my tailbone and when the pouch expands, it feels like someone is rapidly inflating a balloon in my pouch.

A lot of your description sounds like sciatica..meaning that the sciatic nerve is being compressed ,which causes pain down the leg..meaning that it is inflamed. General bed rest and anti-inflamatories help that...it could also be partial lumbao...same treatment but all of them have one thing in common..you are having inflamation in the area, which can weaken your muscle control. Older pouches (meaing that we are getting older too) mean that we may have muscle control issues and need to be doing more specific exercises for the lower back/sacral area...including keigels...to strengthen the whole area...I am finding that my muscles are 'collapsing' with age..getting weaker and not holding things up..doctor wants me doing physio/chiropractic work, pilates, deep breathing/yoga etc...an abitious program but it seems to be helping a bit...then again there are also pouchitis problems and irritable pouch syndrome..
Don't know if any of this helps but....

My pouch is 20 years old now and I had had little problems til the past 3 yrs..I had similar pain as you described, thinking it was sciatic pain, but it ended up being an abscess - groan. I think the inflammation from the abscess was pressing on my sciatic nerve. But if the MRI shows nothing that ends that theory? For your night time accidents perhaps trying eating much earlier in the evening? then your pouch will have little to store. I found also having any vitamins (especially iron tablets) late in the day is a no - no - a definate receipe for a night time accident.Hope you find out what is causing your pain. Wished this site existed years ago - it would have been nice to have other sufferers imput and support....
