When I had pouchitis, I took 3 500 mgs. Cipro for the first 2 or 3 days, then 500 mgs, twice daily for about 10 days. Then one per day for a few days at the end.

I had a recent bout of pouchitis. My doc prescribed cipro 500mg, taking one tab twice daily for 10 days. This was a horse pill!
In addition she prescribed Flagyl at 250mg, taking one tab four times a day for 10 days.
Within a few days, I hardly could go once a day! Almost instant relief except for being constipated! I hadn't had that problem in 18 years or so (ha).

Actually, pills don't "spoil" like food that has reached its expiration; however, the active ingredient(s) can become less potent/effective over time.

500x2 is a normal dose but antibiotics are only effective if you have a full several week perscription.

Blacksabbath: Are you sure? I have a prescription on Cipro 500 mg x 2 for 5 days. Is that course way too short?

5 days sounds short. See my post above [ March 11 ]...for acute pouchitis...10 to 15 days...
